TO MY VALUED PATIENTS AND THEIR FAMILIES

 

I am writing to inform all of you that I am closing my practice. I plan to see patients through Friday, March 29, 2024.

 

I am providing as much notice as possible to give you time to schedule any follow-ups, well visits, or to deal with other issues that you would like to have evaluated before I retire.

 

Also, I urge you to make arrangements for your family’s continuing medical care in the future.  Please call my office if you require any assistance with this transition.

 

I have enjoyed the many years of caring for your children, including their pediatric, adolescent, and young adult needs. I have been honored and privileged to serve you and your family for the past thirty-six years. In that time, I have witnessed many milestones and achievements in your lives, and I have grown fond of each and every one of you. You have entrusted me with your health and well-being, and I am grateful for your confidence and loyalty.

 

I will miss much of the patient care and interaction once I retire, but it is time to explore other aspects of life that I have been unable to do while maintaining my practice. Family issues have also played a role in this decision.

 

My office will be staffed and available to help with transitioning medical records for three months after my retirement date, during which time the normal phone and email contact information will remain active.

 

I want to thank you for your trust and friendship over the years. It has been a pleasure and a privilege to be your pediatrician. I wish you and your family all the best for the future.
